Filling out the paperwork now needs to be completed before we start our sessions. Process for completing forms: Create and Sign in to your Therapy Notes, LLC Client portal via this link:
Telehealth sessions can be conducted using Therapy Notes, LLC client portal. Alternatives include Doxy.me, VSee.com, and Teams from Microsoft Office 365 with whom True Color Counseling, LLC has BAA. If you would like to do the paperwork task before you come to the office, please be sure to download, print out, and fill out the following forms. Thank you for bringing them to your first session. This will allow as much time as possible to focus on you. If I schedule a full 2 hours for an Intake and Treatment plan and to have a real conversation about what’s going on for you.

I value working with clients’ support network be it family, friends, other care providers. This is part of my wholistic, integrative therapeutic medical approach as a counselor. If you feel that you would like to collaborate your session with another provider, simply fill out the following form. If not, that’s ok. Our work together is driven by you. Your choices, your goals … your path.

Call and/or text 828-712-8278. This number is confidential and on 24/7/365. If I do not pick up please contact a crisis line in your area or state. For NM clients: Call 1-800-273-8255 or Call 505-277-3013 or text “HOME” to 741741. OR go immediately to the nearest Emergency Room. Once your are stable and able to do so call me. Please DO NOT text message, email, or contact me electronically when in crisis because I am not able to respond promptly by these mediums.
If an emergency occurs during our session (you fall or faint), know that I have a 2nd cell phone that is used to contact emergency services including 911 contact while I remain on the other line with you.
